	This is an online, automated system maintained by the Federal Maritime Commission to assist the public in preparing and electronically filing your Application for a License as an Ocean Transportation Intermediary (�OTI�). Use of this automated system is optional; alternatively, you may download the written Form FMC-18 and prepare your application using a typewriter. Procedures and requirements established for a typed application remain applicable to your electronic submission.

	Filing Information
	This is an online version of the Form FMC-18. Use of this automated system is optional; alternatively, you may download the written Form FMC-18 and prepare your application manually. To review the Form FMC-18, or to print a copy for use in submitting a typed application,

	Before proceeding with the application, you may wish to collect those documents needed in the application process and create a scanned image of such documents. Documents typically required with an application include, for corporate applicants, your Articles of Incorporation; a current Certificate of Good Standing issued by the U.S. State or jurisdiction in which you do business; a Fictitious Name Statement if you propose to operate under a trade name or �dba�; and a Corporate Resolution or minutes of meeting reflecting appointment of the Qualifying Individual as a corporate officer of your company. Equivalent documents will be required for applicants using other forms of business organization, e.g. Limited Liability Company (LLC), Partnership or Limited Liability Partnerships (LLP).
	In many cases, electronic copies of the above documents may be obtained on-line from government authorities in your respective U.S. State or jurisdiction. To check whether your local Secretary of State makes such corporate records available for online ordering please
	click  here. Scanned documents can be attached electronically as part of your automated filing, using any of the most popular imaging
	formats (such as .jpg, .gif and .pdf).
	You may exit this automated system at any time by first pressing �Save� in order to preserve any information you have prepared. You may return to the saved application at a later date in order to add further information or attach required documentation. Any application not completed after 60 days will be deleted.
	Before Submitting Your Application
	Please check the application to make certain that all applicable parts have been completed and all questions answered or noted �N/A� within those Parts. Electronically attach all supporting documentation (e.g. Articles of Incorporation). Select the �Submit� button to transmit your application. You will receive a confirmation e-mail acknowledging receipt.
	To complete the filing process, you must print and sign a copy of Subpart G. Return your signed copy, together with the appropriate filing fee, to the Bureau of Certification and Licensing, 800 North Capitol Street N.W., Washington, D.C. 20573. Should the applicant/filer change to a paper based submission then applicable paper based fee should be submitted.
