Before proceeding with the application, you may wish to collect those documents needed in the application process and create a scanned image of such documents. Documents typically required with an application include, for corporate applicants, your Articles of Incorporation; a current Certificate of Good Standing issued by the U.S. State or jurisdiction in which you do business; a Fictitious Name Statement if you propose to operate under a trade name or �dba�; and a Corporate Resolution or minutes of meeting reflecting appointment of the Qualifying Individual as a corporate officer of your company. Equivalent documents will be required for applicants using other forms of business organization, e.g. Limited Liability Company (LLC), Partnership or Limited Liability Partnerships (LLP). |

You may exit this automated system at any time by first pressing �Save� in order to preserve any information you have prepared. You may return to the saved application at a later date in order to add further information or attach required documentation. Any application not completed after 60 days will be deleted. |

Before Submitting Your Application |
|
|
Please check the application to make certain that all applicable parts have been completed and all questions answered or noted �N/A� within those Parts. Electronically attach all supporting documentation (e.g. Articles of Incorporation). Select the �Submit� button to transmit your application. You will receive a confirmation e-mail acknowledging receipt. |
|
|
|
|
|
To complete the filing process, you must print and sign a copy of Subpart G. Return your signed copy, together with the appropriate filing fee, to the Bureau of Certification and Licensing, 800 North Capitol Street N.W., Washington, D.C. 20573. Should the applicant/filer change to a paper based submission then applicable paper based fee should be submitted. |
